This position works collaboratively with recipients, their legal representatives, service providers, and other identified supports to develop and coordinate services that promote stability, independence, and personal outcomes. The Case Manager serves as an advocate and resource for recipients, helping them navigate available services, address barriers to care, and make informed choices regarding their treatment and support needs. The position is responsible for developing and maintaining personal service plans, coordinating referrals, monitoring service delivery and outcomes, and ensuring recipients have access to appropriate services and supports. EDUCATION, EXPERIENCE, AND SKILLS REQUIRED Bachelor’s degree in one of the behavioral sciences or a related field, including but not limited to social work, psychology, or nursing, from an accredited college or university. ESSENTIAL FUNCTIONS Develops, implements, reviews, and revises written personal service plans in collaboration with the recipient, the recipient’s legal representative, and others identified by the recipient. Ensures plans are reviewed at least annually and revised when there is a change in the recipient’s status, needs, or goals. Provides ongoing communication and coordination with recipients, legal representatives, service providers, and other identified supports to ensure access to necessary services.Coordinates referrals and monitors the overall delivery, quality, and effectiveness of services. Assists recipients and their legal representatives in making informed choices regarding available services and advocates on behalf of recipients when barriers are encountered, including connecting them with an independent advocate when appropriate. Monitors and evaluates services to ensure personal outcomes are being met and recipients are satisfied with their services and service delivery. Conducts face-to-face monitoring with recipients at least twice annually, completes required documentation, and coordinates with medical assistance facility discharge planners prior to a recipient’s discharge into the community. REQUIRED PHYSICAL ABILITIES While performing the duties of this job, the employee is regularly required to stand; use hands to finger, handle, or feel; and talk or hear. The employee is occasionally required to sit, climb, or balance. Must be able to sweep, mop, shovel, and lift up to 40 pounds. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.  REQUIRED COMMUNICATION, MATHEMATICAL, AND REASONING SKILLS Must possess the skills and abilities necessary to read, write, speak, apply basic mathematical concepts, and use sound reasoning to successfully perform the essential duties and responsibilities of the position. Must be able to perform basic computer operations within the Windows operating system and demonstrate effective problem-solving, critical-thinking, organizational, and multitasking skills.
