Job DetailsJob Location: Worthington, MN 56187Position Type: Full TimeEducation Level: 2 Year DegreeSalary Range: $22.00 - $30.00 HourlyJob Shift: DayThe Care Coordinator serves a dual role: providing front-desk support to link the public and referral agencies to ElevaCare clinical services, and coordinating behavioral and physical health care for clients throughout their treatment experience. This position ensures clients understand and access the services they need, monitors the treatment process including intake, referrals, education, and treatment plan reviews, and works closely with internal and external providers to optimize client outcomes. Using motivational, problem-solving, and client-centered skills, the Care Coordinator supports clients in achieving health goals, navigating care, and maintaining the highest possible quality of life. ESSENTIAL FUNCTIONS Perform front-desk and intake duties: triage phone calls, schedule appointments, obtain demographic and insurance information, complete admissions forms, secure releases of information, and assess client risk using the CSSRS brief. Provide care coordination: manage a caseload, monitor screening tools (PHQ-9/PHQ-9A, GAD-7, CSSRS, SDOH, Gain-SS, Audit-C, Fagerstrom, Biosocial Gambling), and track intake and six-month follow-up data. Support clients through in-person, phone, and virtual outreach, education, motivational counseling, advocacy, and resource linkage. Facilitate communication among clients, providers, families, and staff to ensure coordinated care and treatment plan adherence. Attend and coordinate care team meetings; assist with treatment planning, discharge planning, and transitions from inpatient or residential care. Maintain accurate, compliant clinical records and uphold confidentiality according to ElevaCare, State, and Federal standards. Contribute to quality assurance, safety committees, and program evaluation efforts. Provide coverage for emergencies and support as needed during evening and weekend hours OTHER RESPONSIBILITIES Travel as needed to client locations and facilities. Attend conferences and workshops as directed. Perform other duties as assigned by supervisor. PHYSICAL REQUIREMENTS Regularly required to stand, use hands, talk, and hear; frequently walk and move between departments; occasionally sit, reach, climb, or balance. Occasionally lift/move up to 40 pounds. Visual requirements: close, distance, color, peripheral vision, depth perception, ability to adjust focus. COMMUNICATION & REASONING SKILLS Strong reading, writing, speaking, and problem-solving abilities. Ability to perform basic Windows-based computer operations. EDUCATION, EXPERIENCE & SKILLS REQUIRED Two-year technical college diploma or equivalent education/experience preferred. Experience with office equipment, Microsoft Office Suite, EHR, and billing software. Strong verbal, written, organizational, and interpersonal skills. Knowledge of ElevaCare services, community resources, and client-centered care coordination.
