As our Intake Coordinator, you'll be the crucial link between hospitals, physicians, and families, ensuring every new patient's transition into home health care is fast, accurate, and compassionate.
We're looking for someone who's organized, detail-driven, and genuinely enjoys helping people navigate a stressful moment in their lives. If you have experience in home health, medical office administration, or insurance, and you take pride in doing things right the first time, we want to meet you.
What You'll Do
- Manage the full intake process for new home health patients from referral to admission
- Verify insurance eligibility and benefits, and secure authorizations when required
- Enter accurate patient demographics and information into the EMR
- Serve as a communication hub between patients, caregivers, physicians, case managers, facilities, and referral sources
- Follow up with hospitals and inpatient facilities on pending referrals and care transitions
- Track down outstanding orders with physician offices and marketing staff
- Coordinate staff assignments and scheduled patient visits
- Confirm and document receipt of external visit information
- Maintain organized, accurate patient records and scan documents into the EMR
- Answer and route incoming calls with professionalism and care
- Partner with clinical and administrative teams to keep admissions moving smoothly
What You Bring
- High school diploma or equivalent required
- Home health intake experience strongly preferred
- Solid computer, data entry, and EMR skills
- Familiarity with medical terminology a plus
- Excellent communication and customer service instincts
- Sharp attention to detail and strong organizational habits
- Comfort multitasking and staying calm in a fast-paced environment
