Position Profile:
The Medical Referral Coordinator II is responsible for facilitating referrals to external specialty offices and or hospitals in order to maintain ongoing timely and quality patient care. The Medical Referral Coordinator is part of the medical staff and works as part of a team to contribute to all our patients’ improvement in their health of Bread for the City.Â
Essential Responsibilities:
Manages patients’ referral to hospitals for specialty care:
Maintain referral tracking system by following up with patients and providing support or resources to remove any barriers from receiving services;
Verify patient’s eligibility as applicable and assign correct specialty provider;Â
Ensure that all patient referral appointments have required referral/authorization in place prior to the appointment;Â
Document all appropriate patient information in ECW;
Communicate referrals or insurance issues with Clinic Coordinators and Public Benefits;Â
Listens to and identifies patient needs and provide assistance to patients with dignity and respect;Â
Connects patients to other services at BFC when appropriate;
Supports BFC’s vision, mission, and goals in daily interactions with patients, staff, and volunteers;
Collaborates with Clinic Coordinators to ensure that the Medical Clinic functions quickly and smoothly.
Provides input, collaboration and administrative support in continuously improving the patient experience:
Provides administrative support for specialty clinics;
Supports clinic providers and staff on special projects;
Collaborates with QI & Clinic Operations Manager and other Clinic Coordinators on front office coordination in order to maximize efficiency and capacity to offer our patients quality care;
Provides input, collaboration, and occasional project management in improving clinic flow and operating systems through process improvements and policy creation.
Additional Responsibilities:
Assists with general administration and participates as an active member of the Bread for the City team:
Participates in all-staff meetings, retreats, and trainings;
Contributes to organization-wide projects and initiatives;
Other duties as assigned.
Requirements:
Strong people skills and an ability to work with a wide variety of people, including patients experiencing a variety of physical and/or mental illnesses, and volunteer doctors, nurses, and other health care professionals;
Demonstrated commitment to working for and with low-income people;
Good organizational skills and an ability to juggle multiple responsibilities and projects simultaneously;
Good computer skills;
Must be a team player;
Good listening skills;
Patience, perseverance, and flexibility;
Sense of humor and ability to be calm in the midst of occasional chaos would be very helpful;
Ability to work with volunteers in a professional and collaborative manner;
Bilingual in Spanish or Mandarin is preferred
Familiarity with Google Organization & Productivity Tools (Gmail, Google Docs, Calendar.) and/or ability to learn new technology systems;
Willingness to learn and carry out Bread for the City's commitments to social justice & racial equity.
