Health & Wellness Director – LVN/LPN

We’re looking for a compassionate, experienced Health & Wellness Director (LVN/LPN) to join our Visalia team and provide clinical leadership while helping create an environment where residents feel safe, respected, supported, and truly at home.

What You’ll Do

As our Health & Wellness Director, you’ll lead our care team and oversee the delivery of high-quality resident care. You’ll serve as a clinical resource for team members, residents, and families while partnering with the Executive Director and community leadership team.
Your responsibilities will include:

  • Lead, coach, and develop the care team, including hiring, training, performance management, and ongoing education.
  • Develop and oversee care department schedules to ensure appropriate staffing and consistent quality of care.
  • Assess residents and develop and maintain individualized service plans that reflect changing needs.
  • Monitor changes in resident condition and ensure appropriate assessment, documentation, intervention, and follow-up.
  • Partner with residents, families, physicians, and other healthcare professionals to coordinate care.
  • Oversee medication management systems and ensure medications are administered safely and documentation is accurate and complete.
  • Monitor key areas of resident wellness, including nutrition, weight, skin integrity, mobility, falls, pain management, and overall health.
  • Provide clinical guidance, mentoring, and education to team members, including training related to nursing and dementia care.
  • Participate in resident and family care conferences and help ensure clear, compassionate communication.
  • Maintain accurate resident health records and ensure documentation meets community and regulatory standards.
  • Monitor compliance with applicable state regulations, OSHA requirements, and community policies and procedures.
  • Maintain healthcare supplies and equipment and ensure appropriate inventory and purchasing practices.
  • Serve as a positive role model for professionalism, communication, resident engagement, and behavior management.
  • Help ensure residents receive care that promotes dignity, choice, independence, and respect.
  • Support community leadership and participate in additional management responsibilities as needed.

What We’re Looking For

We’re looking for a healthcare leader who is both clinically confident and people-focused. The ideal candidate will have:
  • Current, active LVN/LPN or RN license in good standing.
  • Previous supervisory or management experience in healthcare, senior living, long-term care, or a related setting preferred.
  • Strong knowledge of senior care and dementia-care practices.
  • Experience training, coaching, and developing team members.
  • Strong assessment, documentation, organizational, and communication skills.
  • The ability to build positive relationships with residents, families, physicians, team members, and community partners.
  • The ability to prioritize and adapt in a dynamic senior living environment.
  • Basic computer skills, including Microsoft Word and Excel.
  • A current driver's license and reliable transportation for occasional off-site assessments and networking.
  • Bachelor’s degree preferred.
  • Ability to meet applicable state licensing and health requirements.
  • Ability to perform the essential functions of the position with or without reasonable accommodation.
  • Successful completion of required pre-employment screening, including criminal background check and TB testing.
