The Team Based Care (TBC) Manager RN independently uses the nursing process to care for a group of patients safely, therapeutically, and efficiently. This role conducts shared patient visits with providers as well as independent RN visits using standardized procedures/protocols and documented workflows. Â The TBC RN collaborates with other team members to coordinate and execute the plan of care. Â Demonstrates initiative in the delivery of patient care in a Team Based Care Model. Â Actions will reflect the Providence core values. Â
Required Qualifications:
- Coursework/Training: Graduate from an accredited school of Nursing.
- Upon hire: California Registered Nurse LicenseÂ
- 2 years Professional Nursing Experience.
- Bachelor's degree in nursing.
- 1 year of experience in ambulatory care nursing.
