Job Summary

We are seeking a Licensed Clinical Social Worker to support individuals experiencing mental health concerns alongside social, environmental, or practical challenges that may affect their overall wellbeing.

The work calls for someone who understands how factors outside the therapy room can influence a client’s health and stability. Strong communication, sound clinical judgment, and the ability to work effectively with other professionals and community organizations are important.

Primary Responsibilities

  • Provide clinical social work support to individuals with behavioral health and psychosocial needs
  • Assess barriers that may affect treatment, stability, or access to care
  • Coordinate with healthcare professionals to support continuity of care
  • Connect clients with appropriate community programs, services, and resources
  • Build and maintain relationships with local agencies and service providers
  • Monitor changes in client needs and coordinate additional support when appropriate
  • Master’s degree in Social Work or a related field.
  • Active LCSW license.
  • Proven experience working with aging populations.
  • Strong skills in clinical evaluations and care planning.
  • Effective communication and team collaboration abilities.
