About the Role:

The Care Navigator (“Navigator”) is an integral member of Community Reach Center’s Aware (“Division”) Division. The Navigator provides navigation and support services to consumers through integrated behavioral healthcare services. The Navigator will utilize Case Management and Care Coordination Model that navigates the use of basic community resources as well as connecting consumers to needed resources through referring, linking, and advocating. Additionally, the Navigator will have other duties and responsibilities as determined from time to time by the Program Manager.

 

Essential Functions: 

  • Ensures all relevant documentation and procedures are followed completely by all persons entering the Drop-In Center along with ensure all CRC information is updated for drop-in center hours, locations, groups, and classes.
  • Complete, organize and fill periodic updates of member information in a timely manner.
  • Collect and enter data in specified locations and as required by CRC and data entry standards, including food pantry, meals, outings/events/ and other services.
  • Facilitate or co-facilitate groups, discussions, or one-on-one services, and ensure that service data is completed and submitted accurately and timely while maintaining confidentiality of consumer records.
  • Answer phones and connects callers to needed information or other employees, including taking and delivering phone messages.
  • Provides customer service approach to consumers receive welcome and direct visitors and members to needed resources within the community and navigates said resources until consumer can maintain independently, acts as a liaison when necessary.
  • Enter new members into our EHR, Credible, and keep insurance updated; assist members with paperwork, general questions, sign in and outs, etc.
  • Keep greeters table and bulletin board organized and updated daily with current and/or relevant information.
  • Cross train other staff to improve efficiency and effectiveness of service delivery.

Qualifications:

  • Requires Bachelor’s Degree in Health Management, Business Administration or another related field
  • 1+ years of experience in a similar management role is required
  • Communication, organization, time management and clinical skills.
  • Bilingual Spanish preferred

Schedule:

On-site position; rotating work schedule of Mon-Fri and Tue-Sat. Work is performed in a sometimes fast-paced, member-facing milieu. Primary duties include welcoming and registering new members, orienting them to programming, and completing and submitting documentation in EHR.

Pay Information:

$24-$27/hr
