The Home Health Physical Therapist (PT) provides skilled physical therapy services to patients in their homes in accordance with physician orders and the established plan of care. This position focuses on restoring function, improving mobility, relieving pain, preventing or limiting physical disability, and promoting greater independence and safety. As part of our interdisciplinary Home Health team, the Physical Therapist works closely with patients, caregivers, physicians, nurses, and other healthcare professionals to provide individualized, high-quality, patient-centered care. Clinical Supervision & Oversight As the Physical Therapist, you will provide clinical supervision and oversight of Physical Therapist Assistants (PTAs) and other assigned therapy personnel in accordance with Medicare Conditions of Participation, Florida licensure requirements, and agency policies. Responsibilities include: Establishing and updating the physician-ordered plan of care based on each patient's needs and progress. Determining appropriate delegation of therapy services and providing clinical direction to assigned therapy personnel. Monitoring patient progress and ensuring therapy services remain skilled, reasonable, and necessary. Reviewing clinical documentation for accuracy, completeness, and regulatory compliance. ✅ Completing required supervisory visits, reassessments, and evaluations. ⭐ Maintaining accountability for the quality, safety, and outcomes of physical therapy services provided under your supervision. What You'll Do Conduct comprehensive initial evaluations to assess patient needs, functional limitations, mobility, home safety, and therapy goals.* Develop individualized treatment plans based on clinical findings, physician orders, and patient-centered goals.* Provide skilled physical therapy interventions, including therapeutic exercise, gait training, balance activities, transfer training, mobility training, and pain-management techniques.* ‍‍ Educate patients and caregivers on home exercise programs, fall prevention, equipment use, safety precautions, and strategies to maximize independence.* Regularly reevaluate patient progress and modify the plan of care as clinically appropriate.* Complete timely and accurate documentation of evaluations, treatment sessions, progress notes, reassessments, and care coordination in accordance with regulatory requirements and agency policies.* Collaborate with physicians, nurses, social workers, therapists, and other members of the interdisciplinary care team regarding changes in patient condition, progress, and ongoing needs.* Recommend and coordinate appropriate durable medical equipment (DME) to support patient safety and functional independence.* ️ Participate in case conferences, interdisciplinary team meetings, and quality improvement activities as required.* ⚖️ Maintain compliance with applicable federal, state, and local regulations, including Medicare Home Health requirements.* Maintain professional knowledge and clinical competency through continuing education, evidence-based practice, and adherence to professional standards.* Why This Role Matters Home Health physical therapy is about more than completing exercises — it's about helping patients safely navigate their everyday lives. Strong clinical assessment, problem-solving, and critical-thinking skills. Knowledge of and experience completing OASIS documentation. Ability to work independently in the home setting while collaborating effectively with the interdisciplinary care team. Excellent communication, interpersonal, and patient/caregiver education skills. Proficiency with electronic medical record (EMR) documentation. ⚖️ Knowledge of Home Health regulatory requirements, Medicare guidelines, and applicable standards of practice. ❤️ Compassion, patience, professionalism, and the ability to build trusting relationships with patients and families. ⏱️ Strong organizational and time-management skills with the ability to manage a community-based caseload effectively. Education & Experience Graduate of an accredited Physical Therapy program required. Current and unrestricted Physical Therapist (PT) license in the State of Florida. Educational requirements must meet applicable Florida licensure standards based on the individual's date and pathway of licensure. At least one year of clinical experience preferred; previous Home Health experience is highly desirable. ❤️ Current CPR/BLS certification. Valid Florida driver's license, reliable transportation, and current automobile insurance as required by agency policy. Physical & Work Requirements This position provides care in patients' homes and requires regular travel throughout the assigned service area. The Physical Therapist must be able to safely perform the essential functions of the position, with or without reasonable accommodation. Travel regularly to patient residences and other locations using a personal vehicle. Sit for periods of time while completing documentation and other computer-based responsibilities. ️ Lift up to 50 pounds at times. Stand, walk, sit, reach, and use hands and arms to safely perform patient-care activities. Access and navigate patient residences and applicable areas of organizational facilities. Possess the visual abilities necessary to safely perform job duties, including close and distance vision, peripheral vision, depth perception, color vision, and the ability to adjust focus. ️ Communicate effectively with patients, caregivers, colleagues, and other healthcare professionals. Wear and properly utilize personal protective equipment (PPE) in accordance with agency policies and applicable safety requirements. Use of Personal Vehicle Use of a personal vehicle is required for this community-based position. Employees must maintain a valid Florida driver's license, reliable transportation, and required automobile insurance coverage throughout employment. Mileage reimbursement is provided in accordance with Company policy. Employees are expected to comply with applicable Florida traffic laws and agency safety policies while conducting Company business, including seatbelt and mobile-device requirements.
