Job Summary and Responsibilities

As a social worker, you will be a compassionate advocate, providing support and guidance to individuals and families facing complex challenges.

Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources.  You’ll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.

 

To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.

 

The Licensed Clinical Social Worker is responsible for performing social work assessments and interventions as needed for inpatients, outpatients and emergency department patients.  The functions of the Licensed Clinical Social Worker includes crisis intervention, patient/family intervention, high risk screening, brief counseling, referring for financial or other identified resource needs, arrange and facilitate family/patient representative meetings with the health care team as needed, arrange post-acute placement on complex discharges and engagement of appropriate agencies or community resources when high-risk patients are identified.

  • Providing developmentally appropriate care for all populations services
  • Advocacy and education, Psychosocial management
  • Patient/Family Care Conferences, Continuity/Transition Management
  • Community Resource Coordination, Performance and Outcomes Management
  • Provide support and social work services to outpatients if directed by Care Coordination leaders.
  • Participate in performance improvement teams and programs as necessary.
  • Clinical Supervision: opportunity to support provision of clinical supervision to staff seeking licensure
  • Provide billable clinical services when appropriate and complete related documents.

Job Requirements

Required Education and Experience:

  • Masters Other from a school of social work accredited by the Council of Social Work Education and
  • At least 2 Years Post-MSW experience. , upon hire and

Required Licensure/Certifications:

  • Licensed Clinical Social Worker: WA, (LICSW/LCSW:WA) upon hire

Preferred

  • Minimum 5-Year Post-MSW healthcare experience
