In this vital role, you will act as a bridge between patients, their support networks, and the healthcare system. You will help educate and empower patients to understand their diagnoses, manage chronic conditions, and build sustainable self-care skills. What You’ll Do: Patient Assistance Use motivational interviews and brief intervention techniques to facilitate patient and patient support person understanding about applicable diagnoses, treatment options, management of chronic conditions, and practices for sustainable self-care.  Help patients and support persons to comprehend and navigate pertinent and available healthcare resources, and assist such persons with development and use of skills that will help to establish positive and supportive relationships and to allow for such navigation to be carried out independently.  Identify and acknowledge patient emotional, physical, and social needs; provide information about pertinent resources that are available to meet such needs; and, as is indicated, assist patients with application for such resources or with connection to a regional Service Integration Team that can help to secure necessities that might include clothing, food, housing, laundry service, or utilities.  Educate patients and support persons about relevant healthcare and outreach topics, and provide such persons with information about appropriate community-based education programs and tools.  Recognize signs and symptoms of patient or support person abuse, promptly report such signs and symptoms to appropriate persons (e.g. healthcare providers, Chief Nursing Officer, or Community Development Executive Director), and, in accord with related Hospital policy and state law, complete mandatory report about such abuse.  Accurately and promptly document and timestamp applicable service provision in the patient health information record.  Help to ensure that Community Development Department information about community resources, patient diseases, and other related matters remains current and relevant.  Performance Improvement Assist with derivation, data collection, and data measurement associated with metrics that are designed to characterize patient health outcomes, quantify patient satisfaction, assess quality of patient services provided, and support plans to improve such provision.  Maintain own knowledge about available resources for patients and patient support persons and remain informed about matters that could significantly impact Hospital operations or organization.  Department Standards Prioritize own orderliness and consistently use a systematic approach to information management.  Comply with Hospital policy and procedure.  Comply with applicable regulations and patient care standards, including standards that relate to patient health information confidentiality.  Educate applicable Hospital staff members about pertinent policy, procedure, regulations, and patient care standards.  Communication Answer electronic mail promptly and appropriately.  Attend semi-monthly Community Health Worker meetings.  Attend external events that are organized by the Hospital and applicable community partners.  Coordinate Department service provision with that of other Hospital Departments and build Hospital staff member awareness about Department services through liaison that takes place as patients access and use other Hospital services.  Know chains of command for the Hospital, Medical Staff, and Department.  Serve as a Hospital representative in interaction with the community and other external entities.  Perform other duties assigned by Director.
