Serves as the liaison for the primary care physician and palliative care providers regarding physicians’ orders for the care plan; responsible for continued communication with physicians and other health care personnel concerning changes in medical treatment and all other aspects of patient care. Provides education and counsel to the patient/family regarding palliative care and their plan of care. Coordinates community resources for patients’ needs as assessed by the palliative care team in conjunction with the social worker. Applies principles of infection control. Follows guidelines for universal precautions in accordance with accepted professional practices. Interdisciplinary Group  Attends, participates, and may facilitate interdisciplinary group conferences [IDG] and special care conferences. Assumes responsibility for establishing and maintaining effective working relationships with team members. Identifies specific patient/caregiver needs and make referral to appropriate team members. Assists the administrative assistant with managing the referral process, provider assignments, provider. Documentation Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards. Reviews and updates pain contracts for each palliative care provider on a weekly basis. Professional and Personal Accountability Demonstrates competency in knowledge and skills. Ensures that VIA Health Partners professional reputation is maintained and projected. Assumes responsibility for professional development and staying abreast of current trends in the healthcare field. Incorporates new information and methods into practice. Maintains current professional licensure, CPR certification and CEU requirements. Maintain productive and professional relations with external agencies and healthcare providers. Exhibits time management skills. Able to plan workday while remaining able to respond appropriately to crisis. May perform other duties as required.   QualificationsMinimum Qualifications A Diploma or associate degree in nursing is required; bachelor’s degree preferred. A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required. The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state’s practice laws and discipline. Current CPR required; CHPN preferred. Minimum 2-year related experience.  Experience in direct care of patients with life limiting illnesses required.  Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical. Computer proficiency is required
