The Care Team Hospice is seeking an experienced and compassionate Registered Nurse (RN) to join our Cleveland-area team in a dedicated 7 On / 7 Off role. This schedule gives you the opportunity to provide meaningful hospice care while enjoying 26 weeks off each year through a consistent every-other-week rotation. During your seven days on, you'll serve as a trusted clinical resource for hospice patients and families during evenings, nights, and weekends. You'll respond to changing patient needs, provide symptom management and crisis support, complete visits when needed, and help families feel confident and supported during some of their most difficult moments. This is an ideal opportunity for an RN who is clinically confident, independent, and comfortable making decisions in the field—and who values having a full seven consecutive days away from work every other week. 7 On / 7 Off Schedule Seven consecutive days on, followed by seven consecutive days off. • Monday–Thursday | 4:30 PM – 8:00 AM • Friday 4:30 PM through Monday 8:00 AM • Every other week completely off • Consistent rotating schedule that allows you to plan your life in advance Territory & Ideal Location Coverage includes: • Lake County • Geauga County • Cuyahoga County • Lorain County • Medina County Ideal Home Base: Independence, OH or within approximately a 30-minute radius, providing a central location within the coverage territory. Our nurses have the autonomy to make clinical decisions while being supported by an experienced interdisciplinary team.   QualificationsWhy This Role? • 7 days on / 7 days off – 26 weeks off each year • Consistent schedule you can plan around • Independence and autonomy in the field • Meaningful one-on-one patient and family interaction • Strong clinical and interdisciplinary team support • Mileage reimbursement for patient visits • Comprehensive benefits and 401(k) with company match • PTO, paid holidays, tuition reimbursement, and additional voluntary benefits Key Responsibilities • Respond to after-hours patient and caregiver needs, providing timely nursing assessment, intervention, and support. • Perform patient visits for symptom management, changes in condition, deaths, admissions, and other urgent clinical needs. • Provide education and guidance to patients and families regarding medications, symptom management, and end-of-life care. • Coordinate with physicians, pharmacies, facilities, caregivers, and interdisciplinary team members to ensure continuity of care. • Complete accurate and timely documentation of assessments, interventions, and care coordination within the EMR. Qualifications • Active Registered Nurse (RN) license in the state of Ohio. • Minimum of one year of nursing experience; hospice, home health, palliative care, oncology, emergency, or critical care experience preferred. • Strong assessment, symptom management, and critical-thinking skills. • Ability to work independently and confidently respond to changing patient needs. • Excellent communication skills and a compassionate approach to patients and families. • Valid driver's license, reliable transportation, and current automobile insurance required.
