Primary Counselor

Primary Counselor Responsibilities

• Conduct comprehensive intake assessments to evaluate clients’ substance use, mental health, psychosocial history, strengths, needs, and readiness for treatment.
• Develop, implement, and regularly update individualized treatment plans based on assessment findings, client goals, and clinical recommendations.
• Provide individual therapy/counseling sessions utilizing evidence-based practices to address substance use disorders, mental health concerns, and co-occurring disorders.
• Facilitate group therapy sessions focused on recovery skills, relapse prevention, psychoeducation, coping strategies, and peer support.
• Conduct family therapy and family education sessions to improve communication, strengthen support systems, and promote healthy recovery environments.
• Complete timely and accurate clinical documentation, including assessments, treatment plans, progress notes, discharge summaries, and other required records in accordance with regulatory and organizational standards.
• Provide crisis intervention and de-escalation services to clients experiencing emotional, behavioral, or psychiatric crises while ensuring client and facility safety.
• Coordinate case management services, including referrals, resource linkage, discharge planning, and collaboration with external providers, community agencies, and support systems.
• Monitor client progress, treatment engagement, and goal attainment, making clinical recommendations and treatment adjustments as needed.
• Participate in multidisciplinary treatment team meetings and collaborate with medical, psychiatric, nursing, and support staff to ensure integrated care.
• Educate clients and families on addiction, mental health, recovery principles, relapse prevention, and community resources.
• Maintain compliance with all applicable ethical standards, confidentiality requirements (HIPAA), licensing regulations, and organizational policies.
• Assist clients with developing aftercare and continuing care plans to support long-term recovery and successful community reintegration.
• Advocate for clients and promote a therapeutic, trauma-informed, and client-centered treatment environment.
• Participate in clinical supervision, staff meetings, training opportunities, and quality improvement initiatives as required.
