Overview

The Clinical Staff Pharmacist demonstrates excellent customer service in that his/her attitude and actions are at all times consistent with the standards contained in the Vision, Mission, and Values of Covenant Healthcare and the commitment to Extraordinary Care for Every Generation. The Clinical staff pharmacist enters orders into the computer for patients Pharmacy is consulted on or is providing dosing for. Consults with nursing, physicians, patients as necessary regarding proper medication use and dosage, potential side effects, and adverse drug reactions. Primary areas of responsibility include antibiotics, TPN, and anticoagulation consults (chemotherapy-Mackinaw). Documents all interventions appropriately in the EPIC computer system. Provides on call service to the Pharmacy department as assigned. Provides Pharmacy services to patients of all ages. Performs other duties as assigned.
Responsibilities

Remains flexible and provides cooperation with staffing requirements as required providing care to our patients

Communicates with all customers (nurses, physicians, patients etc) in a professional, therapeutic, and courteous manner

Has the necessary Pharmacist clinical skills necessary to provide safe, efficient, and effective pharmaceutical care to our patients according to the guidelines as defined by Covenant HealthCare and the Pharmacy Department

Makes clinical decisions and follows through in effective treatment of the patient

Collaborates effectively with all health care disciplines to facilitate patient outcomes based on safe Pharmacy practice

Enters medication orders efficiently utilizing the EPIC computer application taking care to accurately and safely dose patients

Ability to provide care on consults generated to Pharmacy by physicians for areas including but not limited to antibiotics, pain medications, TPN management, IV anticoagulation (Chemotherapy-Mackinaw)

Assist the purchasing technicians as required to obtain and maintain inventory of needed pharmaceuticals

Actively participates in quality management including the documentation of pharmacy interventions

Updates and maintains knowledge, skills and competencies related to area of practice and ages of patient population

Participates in unit meetings and activities which support unit function and the overall goals of the organization

Monitor technicians� preparation and dispensing as necessary

Maintains professional development through seminars, workshops, in-service activities, and current professional literature

Other duties as assigned



Qualifications

EDUCATION/EXPERIENCE
Current Michigan licensure to practice as a Pharmacist
PharmD preferred. 1 yr. Residency preferred.
Maintains professional development through seminars, workshops, in-service activities, and current professional literature.
Minimum of 3 years experience in a hospital setting preferred
Previous clinical pharmacy experience preferred

KNOWLEDGE/SKILLS/ABILITIES
Computer competency including but not limited to EPIC and Lotus notes
Ability to utilize all reference material needed to provide pharmaceutical care.
Has the training and abilities necessary to be a Clinical Pharmacist
Excellent oral and written communication skills.
Must be able to work well with all our customers and able to be tactful in often hectic and stressful situations.
Pharmacist skill set that emphasizes clinical activities
Sound judgment in handling confidential or sensitive issues and material.
Able to flex work schedule in order to meet department and staffing needs

WORKING CONDITIONS/PHYSICAL DEMANDS
Ability to maintain punctual attendance consistent with the ADA, FMLA, and other federal, state, and local standards.
Frequent lifting up to 10 lbs.
Frequent standing, walking, sitting, talking, hearing, near vision, midrange vision, visual accommodation, color vision and field of vision.
Occasional lifting up to 25 lbs.
Occasional lifting, carrying, pushing, pulling, climbing, balancing, stooping, kneeling, crouching, squatting, twisting, reaching, handing, far vision and depth perception.
