Occupational Therapist - PACE Program

The Occupational Therapist provides comprehensive occupational therapy evaluations, treatments, and interventions for PACE participants in day center, nursing home, and home-based settings. This role supports older adults in maintaining independence, safety, and quality of life through restorative, preventative, and maintenance therapy services.

Key responsibilities include conducting initial and ongoing assessments, developing individualized care plans, implementing therapeutic interventions, and collaborating with the Interdisciplinary Team to achieve participant-centered goals. The Occupational Therapist evaluates functional abilities, cognitive and sensory needs, activities of daily living (ADLs), and home safety requirements. The position coordinates home modifications, recommends adaptive equipment, and assists with the ordering, inventory, and maintenance of durable medical equipment.

Additional responsibilities include providing caregiver and staff education, documenting participant progress in the medical record, and monitoring treatment effectiveness. The Occupational Therapist supervises and evaluates Certified Occupational Therapy Assistants (COTAs), supports competency assessments, and contributes to departmental quality and operational goals.

The successful candidate demonstrates strong clinical judgment, organizational skills, and a commitment to serving frail and elderly populations. This role requires effective communication, collaboration, and the ability to adapt to changing participant needs while upholding organizational values and regulatory standards.

Qualifications:

  • Bachelor's degree in Occupational Therapy from an accredited program.
  • Current state licensure as an Occupational Therapist.
  • Minimum one year of experience working with frail or elderly populations preferred.
  • Strong assessment, treatment planning, and documentation skills.
  • Experience with interdisciplinary care teams and caregiver education.
  • Proficiency with electronic medical records and Microsoft Office applications.

This position requires travel within the service area and the ability to work effectively with diverse participants, families, caregivers, and healthcare professionals to promote independence, safety, and optimal functional outcomes.
