We are seeking a compassionate and experienced Registered Nurse (RN) Wound Nurse to join our clinical team. The Wound Nurse is responsible for assessing, treating, and monitoring residents with wounds, pressure injuries, skin conditions, and related clinical needs. This position works closely with physicians, nursing staff, therapy teams, and facility leadership to promote healing, prevent skin breakdown, and ensure compliance with regulatory standards.

Essential Duties and Responsibilities

  • Conduct comprehensive wound and skin assessments upon admission and as clinically indicated.
  • Develop, implement, and update individualized wound care treatment plans.
  • Monitor and document wound healing progress and communicate changes to physicians and interdisciplinary team members.
  • Perform wound treatments and dressing changes in accordance with physician orders and facility protocols.
  • Lead facility-wide skin integrity and pressure injury prevention programs.
  • Ensure accurate and timely documentation within the electronic medical record.
  • Educate residents, family members, and staff regarding wound prevention and treatment strategies.
  • Collaborate with nursing leadership to identify and address wound care trends and opportunities for improvement.
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Maintain compliance with state, federal, and facility regulations.

Qualifications

Required

  • Current Registered Nurse (RN) license in the state of employment.
  • Minimum of one (1) year of nursing experience.
  • Strong clinical assessment and documentation skills.
  • Knowledge of infection control practices and skin integrity management.
  • Excellent communication and organizational skills.

Preferred

  • Previous wound care experience in a skilled nursing or long-term care setting.
  • Wound Care Certification (WCC, WOCN, or similar) preferred or willingness to obtain.
  • Experience with PointClickCare or other electronic health record systems.
