Position Summary As a Palliative Care Nurse Practitioner (NP) with Impact Health PC in Hartford, CT, you will provide compassionate, patient-centered care to individuals with serious and complex illnesses within skilled nursing facilities. You will assess symptoms, develop individualized treatment plans, support goals-of-care discussions, and collaborate closely with patients, families, facility staff, physicians, and interdisciplinary care teams. Key Responsibilities Provide compassionate, person-centered palliative care using evidence-based practices. Conduct comprehensive assessments and develop individualized treatment plans. Manage symptoms and provide ongoing follow-up for patients with serious and complex illnesses. Support goals-of-care discussions with patients and families. Collaborate with physicians, facility staff, and interdisciplinary care teams. Educate patients, families, and facility staff regarding palliative care and treatment plans. Complete timely and accurate clinical documentation. Maintain compliance with facility, state, and federal regulations.
