We are seeking a vivacious, proactive Remote RN Case Manager to join our multidisciplinary team and help us provide comprehensive, compassionate care to patients with complex chronic health conditions.

What You’ll Do (Primary Accountability)

As our Remote RN Case Manager, you will serve as a vital clinical partner to our physicians, medical providers, and back-office teams. You will manage provider panels, coordinate follow-up care, and act as a champion for patient self-management—helping patients avoid preventable hospital stays and thrive in their daily lives.

Key Responsibilities:

  • Multidisciplinary Collaboration: Communicate seamlessly with physicians, nurses, therapists, and social workers following established disease management protocols.

  • Patient Advocacy & Education: Provide individualized screening, disease education, care planning, stabilization, and self-management support.

  • Triage & History Taking: Utilize sharp clinical judgment and critical thinking to triage patient needs and collect detailed medical and social histories.

  • Registry Management: Maintain and monitor patient registries across provider panels to track and support individuals with complex, chronic conditions.

  • Care Plan Evolution: Continuously review and update plans of care with the provider team to quickly address emerging patient needs.

  • Staff Support: Serve as a trusted clinical resource and provide guidance to back-office healthcare staff.

What We’re Looking For

  • Licensure: Active Registered Nurse (RN) license in the State of Washington (Required).

  • Education: Associate or Bachelor’s degree in Nursing (ADN/BSN).

  • Clinical Experience: Minimum of 2 years of recent clinical RN experience (or equivalent case management experience) in an outpatient, inpatient, or clinic setting. Experience managing chronic conditions is highly preferred.

  • Tech Capabilities: High-speed, highly reliable internet connection (no intermittent drop-offs) and comfort utilizing Electronic Health Record (EHR) systems and registry tools.

  • Personality & Mindset: Creative, adaptable, patient, and full of energy! You use critical thinking and visionary problem-solving to navigate complex clinical and social dynamics over the phone/video.

PROFESSIONAL & TECHNICAL KNOWLEDGE

Job duties require knowledge and training in the field of nursing typically acquired through a formal Associates or Bachelor degree. Job experience working with patients with chronic conditions is desired. Two years of recent clinical experience or equivalent case management as an RN working in a clinic/inpatient/outpatient setting preferred.

LICENSES AND CERTIFICATIONS

Must have an active Registered Nurse licensure with the State of Washington

Must have a strong, reliable internet connection that does not lose speed or service intermittently.
