Job DetailsJob Location: Canyon Utah Home Care & Hospice - Ogden - South Ogden, UT 84403On-Call RN – Job Requirements Required Qualifications Current, unrestricted Registered Nurse (RN) license in the state of practice. Current CPR/BLS certification. Minimum 1 year of home health nursing experience preferred. Strong clinical assessment and critical-thinking skills. Working knowledge of home health regulations, Medicare requirements, OASIS, and plan-of-care management. Comfortable working independently and making appropriate clinical decisions. Reliable access to a phone and ability to respond promptly to calls/texts. Valid driver’s license, reliable transportation, and appropriate auto insurance. On-Call Responsibilities Provide after-hours clinical support to home health patients, caregivers, and field clinicians. Triage patient and clinician calls and determine the appropriate level of intervention. Provide clinical guidance to field staff regarding changes in condition, symptoms, medications, falls, wounds, and other urgent concerns. Determine when a patient requires emergency services, physician notification, an urgent visit, or routine follow-up. Contact physicians/providers as needed and document all clinical communications. Coordinate urgent or PRN visits when clinically appropriate. Assist clinicians with questions regarding orders, medications, procedures, and patient care. Review and respond to after-hours hospital/ER notifications when necessary. Communicate important overnight/weekend issues to the appropriate clinical leadership for follow-up. Maintain accurate and timely documentation of all calls, interventions, notifications, and follow-up. Follow agency policies, physician orders, and applicable regulatory requirements. Ideal Candidate Calm and confident in urgent situations. Excellent communication and triage skills. Able to prioritize multiple calls and determine what requires immediate attention. Comfortable making independent clinical decisions within scope of practice. Familiar with home health workflows and the needs of field clinicians.
