Job Summary
- Evaluate and verify medication orders for Outpatient Infusion Center
- Coordinate care transitions between acute inpatient stays and outpatient infusion center appointments
- Select appropriate therapy for patients based on clinical factors and reimbursement parameters
- Evaluate patient-specific information, such as labs, imaging, disease state, and other medication therapy to ensure patient safety and to optimize efficacy for patients receiving infusions
- Communicate effectively with medical and nursing staff to ensure appropriate therapy for infusion center patients
- Provide counseling on outpatient treatment side effects as needed
- Serve as a liaison for patient assistance programs
- Complete reimbursement analysis on new medication requests, and update as payer changes arise
- Optimize infusion center formulary based on medication costs, reimbursement, and clinical factors
- Serve as the primary clinical resource for hospital-based physicians, oncology teams, and infusion center nursing staff
- Create, edit, and maintain various order sets to be used for the infusion center
Qualifications
Education
- Bachelor of Science in Pharmacy or a Pharm D degree from an accredited college of pharmacy.
- Pharmacy Residency preferred, but not required.
Experience
- Preferred experience working with MediTech EMR.
- Preferred familiarity/experience working with infusion center reimbursement models.

Licensure/Certification
- Licensed and in good standing with the OK State Board of Pharmacy.

Compensation/Benefits
Â
Up to $81.87/hr. based on previous work experience.
