JOB TYPE: Family Case Manager (Full Time)

LOCATION: New Castle, DE

QUALIFICATIONS:

  • Bachelor’s in Social Work or related field.
  • At least 1 consecutive year of child welfare experience
  • Experience working with children and families in social services context
  • Organization and strong time management skills
  • Valid driver's license with properly registered and insured vehicle
  • Proficient in Microsoft Office

JOB DESCRIPTION:

1. Assessing the family and child(ren) using the Family Strengths and Needs Guide and Child Strengths and Needs Guide while helping the family develop a plan to mitigate the safety threats when safety threats are identified.

2. Developing a Family Service Plan with the family that addresses the identified needs

related to risk and safety of a caseload of up to 12 families.

3. Reviewing the progress on the Family Service Plan with the family.

4. Assisting the family in building a support network. and facilitating Family Team Meetings.

5. Assisting the family in managing crisis situations and referring families to resources or services.
