Medically Tailored Meals
Reports to: Kitchen Manager
Location: Bronx, NY/Hybrid
Full-time, hybrid $80,000 to $100,000 per year
Join a growing New York City medically tailored meal provider as our in-house Registered Dietitian. You will own clinical eligibility screens, meal approvals, therapeutic menus, and telehealth nutrition counseling for Medicaid members referred through New York State's Social Care Network program. Remote clinical work plus regular time in our NYC kitchen.
About Kommissary:
Kommissary is a New York City food service provider preparing fresh, medically tailored meals for Medicaid members referred through New York State's Social Care Networks (SCNs), including SOMOS Healthcare Providers, under the state's 1115 Medicaid waiver program. Our meals help New Yorkers living with diabetes, heart disease, kidney disease, cancer, and other chronic conditions, as well as pregnant and postpartum members. This role leads our clinical nutrition work.
The role:
You will run the nutrition screens and assessments that qualify SCN members for medically tailored meals, determine and approve the right meal track for each member, set our therapeutic menu standards, and provide nutrition counseling to members. You will work closely with our culinary, operations, and member services teams and with SCN navigators and referring providers.
Screens, counseling, and documentation can be done remotely. You will also spend regular time on site at our New York City kitchen for menu development, nutrient analysis, and production quality review.
Duties:
Conduct nutrition screens and assessments for SCN members referred for medically tailored meals, confirming clinical eligibility for conditions such as diabetes, heart failure, kidney disease, hypertension, cancer, and pregnancy or postpartum, and identifying allergies, cultural and religious food preferences, and texture modifications.
Determine, document, and approve the medically appropriate meal track for each member (carbohydrate-controlled, renal, cardiac, high-protein, gestational, and so on), and reassess at set intervals and at renewal.
Provide the registered dietitian approval required for medically tailored meals under the New York State SCN program, and keep complete, audit-ready documentation in our systems and the SCN referral platform.
Coordinate with SCN navigators, referring clinicians, and our member services team on intake, status changes, and transitions off service, including connecting members to WIC and SNAP.
Own the therapeutic menu standards for each meal track, run nutrient analysis, and confirm menus meet condition-specific targets for calories, macronutrients, sodium, potassium, phosphorus, added sugar, and fiber.
Work with our culinary team on recipes, substitutions, portioning, labeling, and cultural relevance, and maintain the nutrition specifications and quality checks used by production staff.
Deliver individual and group nutrition counseling and education by telehealth and in person, and develop plain-language, culturally relevant education materials.
Serve as our subject matter expert on New York State HRSN nutrition requirements, support reporting, quality assurance, and audits, and help build protocols and training as the program grows.
Requirements:
Registered Dietitian or Registered Dietitian Nutritionist (RD/RDN) credential in good standing with the Commission on Dietetic Registration.
New York State Certified Dietitian-Nutritionist (CDN), or eligible and willing to apply upon hire. Kommissary will reimburse the application fee.
Bachelor's degree in nutrition, dietetics, or a related field.
At least two years of experience in clinical, community, or food service dietetics, including medical nutrition therapy for conditions such as diabetes, cardiovascular disease, and kidney disease.
Experience conducting nutrition assessments and documenting in an EHR, care management, or referral platform.
Experience serving Medicaid, immigrant, or low-income communities, and strong written and verbal communication skills.
Based in the New York City area and available to work from our NYC kitchen on a regular weekly schedule.
Preferred: bilingual English and Spanish, since many of the members we serve are Spanish speaking. Experience with medically tailored meals, Food Is Medicine programs, or Medicaid HRSN services (SCNs, Unite Us, findhelp, or similar). Large-scale menu planning and nutrient analysis software such as ESHA/Trustwell or Nutritics. ServSafe or NYC Food Protection Certificate.
Why this role:
You will have real ownership of menus, protocols, and how the clinical program is built. No inpatient coverage and no weekend or holiday rotation. Hybrid schedule, and we support your CDR continuing education.
Compensation and benefits:
Salary of $80,000 to $100,000 per year, depending on experience and credentials. Full-time, salaried, hybrid.
