  • Monitor residents for changes in behavior, health, or well-being and report concerns promptly
  • Follow individualized care plans, safety plans, and established residential procedures
  • Maintain a clean, safe, comfortable, and supportive home environment
  • Complete required overnight checks, documentation, shift notes, and incident reports accurately and timely
  • Prepare the home and residents for the upcoming day as needed
  • Communicate important information and changes to the incoming staff at shift change
  • Respond appropriately to emergencies and follow established crisis and emergency procedures

  • Strong communication and documentation skills
  • Ability to follow individualized care plans, policies, and safety procedures
  • Ability to work independently while also communicating effectively with the care team
