Successful candidates will have the following:

Medicaid application experience
Experience with social work in a LTC setting
Compassion for an underserved population
Ability to work effectively as team
Excellent communication skills

Your day to day:

You'll provide assistance in caring for our residents by ensuring care plans meet their needs and they and their families are communicated with effectively.You'll work under supervision of our Social Services Director to ensure compliance with current state, federal, and local standards.
