- Collaborate effectively with interdisciplinary teams to coordinate discharge planning, case management, ensuring seamless transitions of care.
Skills
- Strong knowledge of nursing practices across multiple specialties including critical care, emergency medicine, geriatrics, pediatrics, internal medicine, and long-term care environments.
- Familiarity with advanced clinical procedures such as tube feeding management,
- Excellent caregiver communication skills to educate patients and families about treatment plans while demonstrating empathy and professionalism.
