Description


 The Care Coordinator plays a crucial role in ensuring that Senior1Care delivers exceptional, personalized care to clients and their families while providing caregivers with the support they need to be successful. The Care Coordinator is responsible for coordinating client care, managing and coaching caregivers, maintaining high-quality service, and building strong relationships with clients, families, and caregivers.


By understanding the unique needs of each client and supporting caregivers through effective scheduling, communication, training, and resources, the Care Coordinator helps ensure a seamless and professional care experience.


Key responsibilities include:

  • Assess client needs and communicate with caregivers to ensure each client receives personalized support and established care plans are followed.
  • Serve as a primary point of contact for clients and their families, providing regular updates and promptly addressing questions, concerns, and changes in care needs.
  • Assign caregivers to clients based on care needs, personality, availability, and proximity.
  • Manage caregiver availability, open shifts, schedule changes, and call-offs to help ensure continuity of care.
  • Provide ongoing support and oversight to caregivers to ensure Senior1Care's standards of care are consistently met.
  • Build strong relationships with clients, families, and caregivers to anticipate needs and ensure satisfaction.
  • Communicate changes in client needs and care plans clearly and promptly to caregivers.
  • Maintain accurate and up-to-date client documentation, including care updates, changes in condition, and other relevant care information.
  • Conduct regular oversight visits to assess client needs and monitor the quality of care being provided.
  • Perform caregiver meet-and-greets and introductions at client locations to help create successful client-caregiver relationships.
  • Proactively identify potential challenges and develop creative solutions to prevent disruptions in care.
  • Manage client and caregiver concerns or conflicts professionally, working toward solutions that maintain strong relationships and high standards of care.
  • Facilitate caregiver training assignments and help ensure compliance with company, legal, and licensure requirements.
  • Protect client privacy and dignity in accordance with HIPAA and company policies.
  • Participate in the night and weekend on-call rotation, providing timely support for urgent client and caregiver needs or unexpected situations.
  • Assist with caregiver interviews as needed and provide input on prospective hires.
  • Collect and document feedback from clients, families, caregivers, and referral sources to support continuous improvement in the quality of care and overall client experience

Requirements


  • Bachelor’s degree in a relevant field and professional experience in healthcare, mental health, home care, or a related field preferred.
  • Excellent written and verbal communication skills, with the ability to communicate effectively with clients, families, caregivers, and team members.
  • Strong relationship-building and interpersonal skills.
  • Ability to defuse conflicts and disagreements and develop practical, workable solutions.
  • Strong organizational skills, with the ability to multitask, prioritize competing needs, and respond effectively in a fast-paced environment.
  • Solid computer skills, including Microsoft Office, with the ability to learn scheduling, documentation, and other technology platforms.
  • Excellent phone etiquette and professional communication skills.
  • Strong attention to detail and a commitment to accurate documentation.
  • Compassionate, kind, and professional demeanor.
  • Dependable and responsive, with the ability to occasionally participate in a night and weekend on-call rotation.
