What You'll Do

As a Team Lead, you will:

• Provide first-line supervision, coaching, mentoring, and performance support to direct care staff
• Organize, schedule, and distribute work assignments among team members
• Communicate program policies, procedures, and expectations to staff
• Maintain a direct service caseload and provide direct support as needed
• Coordinate and oversee medical appointments and follow-up care
• Communicate with nurses, physicians, guardians, family members, and other stakeholders regarding individual needs
• Participate in person-centered planning meetings and support the implementation of individualized plans
• Ensure required documentation, progress notes, and monthly reviews are completed accurately and on time
• Coordinate communication with service coordinators and interdisciplinary teams
• Prepare reports and maintain documentation in compliance with agency and regulatory requirements
• Support quality improvement initiatives and ensure regulatory compliance
• Perform other duties as assigned

Qualifications

Required

• Current Certified Psychiatric Rehabilitation Practitioner (CPRP) Certification
• Valid Driver's License
• Bachelor's Degree in a Human Services field such as Sociology, Special Education, Rehabilitation, Nursing, Counseling, Psychology, or a related field

OR

• Associate Degree with at least three (3) years of experience supporting individuals with intellectual or developmental disabilities

OR

• High School Diploma/GED with at least five (5) years of relevant experience

• Strong communication, leadership, and interpersonal skills
• Ability to work collaboratively with staff, families, guardians, and community partners

Preferred

• Previous supervisory or team leadership experience
• Experience supporting individuals with behavioral health or intellectual/developmental disabilities
• Knowledge of person-centered planning and regulatory compliance requirements

Schedule

• Full-Time
• Schedule based on program needs
• Flexibility for evenings, weekends, and location changes may be required

Physical Requirements

• Ability to exert up to 50 pounds of force occasionally and perform physical job duties as needed
• Ability to sit, stand, walk, climb stairs, reach, kneel, stoop, crouch, and perform repetitive motions
• Ability to travel to appointments, meetings, and community activities as required
• Ability to perform essential job functions with or without reasonable accommodation
