What You'll Do
As an Advanced Practice Provider, you will:
- Provide comprehensive primary care services to an assigned patient panel in collaboration with physicians and the care team.
- Conduct patient assessments, develop individualized care plans, and implement appropriate treatment interventions.
- Diagnose and manage acute and chronic health conditions using evidence-based practices.
- Interpret diagnostic testing, including laboratory and imaging results.
- Prescribe medications and treatment plans within your scope of practice.
- Educate patients and families on disease management, preventive care, symptom management, and healthy lifestyle choices.
- Coordinate care across providers, specialists, caregivers, home health agencies, and community resources.
- Support advanced care planning and palliative care discussions for patients with chronic or life-limiting illnesses.
- Help address quality measures, risk adjustment (HCC) documentation, and patient outcomes initiatives.
- Reduce avoidable emergency department utilization through proactive care management.
- Ensure accurate, timely documentation and billing in compliance with organizational and regulatory standards.
- Collaborate closely with physicians and consult on complex diagnoses or treatment decisions as needed.

Qualifications
Required
- Current, unrestricted Florida license as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA).
- Master's Degree in Nursing (for APRN candidates) or graduate degree from an accredited Physician Assistant program.
- Ability to practice within Florida scope-of-practice guidelines.
- Strong clinical judgment, communication, and patient education skills.

Preferred
- Two or more years of primary care, geriatrics, chronic care management, or related clinical experience.
- Current CPR/BLS certification.

$108,466.00to
$162,698.00Â
The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
