RN (Home Care)Job Description
Registered Nurse is responsible for providing nursing care in accordance with state licensure status and in compliance with policies and procedures, applicable health care standards and governmental laws and regulations.
Responsibilities
Assists with implementation of the plan of care under the direction and supervision of the registered nurse., Evaluates patient/client needs and makes appropriate referrals to the RN Case Manager for other services., Orders all supplies patient specific to meet patient needs., Other duties as assigned to assist with overall operations of the agency., Participates in-home health care case conferences, staff meetings and in-services as required., Prepares documentation in patient/client clinical record to reflect delivery of quality and safe home health care. Documentation is submitted timely and according to policies/procedures., Provides for patient/client and family input into care planning and discharge planning. Documents collaboration with care plan and discharge planning., Provides information regarding equipment and supply needs to patient/client/family and makes appropriate referral., Works in the patient home, independently within the standards of the Nurse Practice Act and following physician orders, to coordinate the patients’ care in a safe manner.Requirements
High School (Required)Automobile Insurance - Sarah Bush Lincoln, BLS or CPR certification (within 90 days of hire) - Sarah Bush Lincoln, Driver's License - State of Illinois, RN-Registered Nurse - Illinois Department of Financial and Professional RegulationCompensation
