Job DetailsJob Location: Signio Specialty Pharmacy - Rockaway Township, NJ 07866Salary Range: $63.00 - $65.00 HourlyPosition Summary The Specialty Pharmacist is responsible for providing comprehensive clinical pharmacy services to patients receiving complex, high-cost, and/or high-risk specialty medications. Specialty Pharmacist will collaborate closely with patients, caregivers, prescribers, nurses, pharmacy technicians, insurance plans, manufacturers, and other healthcare professionals to optimize outcomes and deliver an exceptional patient experience. Essential Duties and Responsibilities Review specialty medication prescriptions for clinical appropriateness, including dosing, drug interactions, contraindications, allergies, and therapeutic duplication. Perform comprehensive medication reviews and identify potential drug therapy problems. Counsel patients and caregivers on specialty medications, including administration, storage, missed doses, adverse effects, precautions, and expected outcomes. Conduct new-patient onboarding and medication initiation assessments. Provide ongoing clinical monitoring for medication effectiveness, safety, adherence, and adverse events. Follow up with patients at appropriate intervals based on medication, disease state, and clinical needs. Identify adherence barriers and develop individualized interventions to improve medication adherence and persistence. Coordinate with prescribers and other healthcare providers regarding therapy recommendations, adverse events, laboratory monitoring, and changes in treatment. Support the medication access process, including benefits investigations, prior authorizations, appeals, and manufacturer or financial assistance programs. Review clinical information and documentation required to support coverage of specialty medications. Communicate with insurance plans, prescriber offices, and patients to resolve medication access issues. Coordinate timely dispensing and delivery of specialty medications. Document patient assessments, interventions, counseling, clinical recommendations, and outcomes accurately in the pharmacy management system and/or electronic health record. Maintain appropriate communication with patients throughout the specialty medication journey. Participate in medication management programs, disease-state management, and quality improvement initiatives. Assist with development and implementation of pharmacy protocols, clinical programs, and standard operating procedures. Monitor and report medication-related adverse events and medication errors according to organizational policies and applicable regulations. Maintain current knowledge of specialty medications, clinical guidelines, treatment protocols, safety information, and emerging therapies. Support specialty pharmacy accreditation and quality initiatives, including applicable patient management and quality measures. Maintain compliance with federal and state pharmacy laws, regulations, payer requirements, and organizational policies. Serve as a clinical resource and mentor to pharmacy technicians, interns, pharmacists, and other team members. Participate in staff education and training related to specialty medications and clinical programs. Perform other duties as assigned. Patient Care Responsibilities The Specialty Pharmacist will provide patient-centered care throughout the specialty medication process, including: Medication initiation: Assess patients before starting therapy and provide comprehensive medication education. Medication access: Help patients navigate insurance requirements and financial assistance opportunities. Medication management: Evaluate therapy for appropriateness, safety, and effectiveness. Adherence support: Identify and address barriers that may prevent patients from taking medications as prescribed. Clinical monitoring: Assess treatment response, adverse effects, laboratory values, and other relevant clinical parameters. Ongoing follow-up: Maintain regular communication with patients and healthcare providers to promote continuity of care. Transitions in therapy: Support medication changes, discontinuations, and transitions between therapies when appropriate.   QualificationsQualifications Required Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program. Active and unrestricted pharmacist license in the applicable state. Strong knowledge of pharmacotherapy, medication safety, and specialty medications. Excellent verbal and written communication skills. Strong clinical judgment and problem-solving abilities. Ability to work effectively in a fast-paced, patient-centered specialty pharmacy environment. Strong organizational skills and attention to detail. Proficiency with computers, electronic health records, and pharmacy management systems. Preferred Previous specialty pharmacy experience. Ambulatory care, health-system, or clinical pharmacy experience. Residency or fellowship training. Board certification such as BCPS, BCACP, or a disease-state-specific BPS certification. Experience with specialty pharmacy accreditation requirements. Experience with prior authorizations, appeals, benefits investigations, and manufacturer assistance programs. Experience with medication adherence and patient management programs. Experience working with complex chronic or rare disease therapies.
