JOB PURPOSE:

 

  • Obtain post admission authorizations
  • Act as the liaison between the physicians, hospitals and Insurance Case Managers
  • Assist in the development of Care Plans for medically complex residents
  • Manage and coordinate caseload upon admission, throughout stay/discharge
  • Timely communication with updating external physicians/Case Mng             
  • Communicate patient care needs with the IDT
  • Support patients and their families
  • Attend weekly Medicare Meetings
  • Evaluate for denials and assist with appeals
  • Coordinate discharges including transport, meds, and referrals for home care.
  • Follow up with patient and/or family post discharge
  • Other duties as assigned

 

 

POSITION REQUIREMENTS:

  • Three years clinical case management experience with at least one in long term care required.
  • Advanced communication, problem solving and interpersonal skills.
  • Knowledge of Medicare and Medicaid regulations in long term care required.
  • Licensed Nurse or Licensed Social Worker preferred
