Registered Nurse Care Coordinator (RNCC) – Endocrinology, Children’s Hospital

Help children and families navigate complex endocrine conditions with confidence, continuity, and expert nursing guidance. 

This Pediatric Endocrinology RN Care Coordinator position blends clinical triage, patient and family education, chronic disease management, and interdisciplinary care coordination. You will practice with meaningful autonomy while building trusted relationships with patients, guardians, providers, social workers, and clinical teams across ambulatory and inpatient settings. 

The position offers a full-time, salaried Monday through Friday schedule from 8 a.m. to 5 p.m., with no call or weekends and holidays off. Hybrid flexibility may be available after onboarding and demonstrated independence in the role. 

The Role 

As an RN Care Coordinator, you will serve as a consistent clinical resource for children with endocrine disorders and the families supporting them. Much of your day will center on triage and teaching, applying strong clinical judgment to assess needs, communicate recommendations, reinforce treatment plans, and help families understand how to manage chronic conditions beyond the clinical encounter. 

This role is primarily based in office and clinical settings. You will not routinely attend clinic visits, but may join when an individual patient’s needs call for additional nursing support. Education may also take place in the inpatient setting, giving you the opportunity to promote continuity across care environments. 

You will: 

• Provide endocrine-focused triage and education for pediatric patients and their guardians. 

• Coordinate care and communicate closely with providers, social workers, nurses, and other members of the interdisciplinary team. 

• Deliver individualized teaching in ambulatory and inpatient settings based on patient and family needs. 

• Support chronic disease management through clear communication, relationship-building, and consistent follow-through. 

• Recognize changes in clinical status, prioritize concerns, and help patients and families navigate next steps. 

• Collaborate with the diabetes RN Care Coordinator when patient needs or workflows overlap. 

• Identify opportunities to strengthen processes and contribute practical solutions that improve the patient, family, and team experience. 

• Balance independent decision-making with highly collaborative team-based care. 

This opportunity is well suited to an experienced RN who brings pediatric, ambulatory, endocrinology, diabetes, or care coordination experience and is known for critical thinking, initiative, communication, and patient-centered problem-solving.
