UVA Health Family Medicine Crozet is seeking an experienced Advanced Practice Provider to deliver comprehensive, relationship-based care in an engaged rural community. This opportunity offers the variety of full-spectrum family medicine, the resources of a multidisciplinary care model, and a predictable Monday through Friday schedule with no weekends or holidays.Ā Ā

The RoleĀ 

In this outpatient role, you will care for patients from infancy through older adulthood, managing preventive, acute, and chronic healthcare needs while developing trusted, long-term patient relationships. You will practice alongside physicians, APPs, and an extensive network of clinical and support professionals who collaborate to address the whole patient.Ā 

Ā 

The schedule includes eight patient-care sessions each week plus approximately eight hours of protected administrative time for inbox management, documentation, and related responsibilities.Ā Ā 

Ā 

You will join a collaborative clinical group that includes physicians and APPs, with access to multidisciplinary expertise within the practice. The clinic provides a structured APP onboarding and ramp-up experience that is individualized according to your background, supporting progression toward independent management of a full patient panel.Ā

Your work may include:Ā 

  • Providing comprehensive primary care across the lifespanĀ 

  • Evaluating and managing acute and chronic conditionsĀ 

  • Delivering preventive care, wellness services, and patient educationĀ 

  • Providing women’s health servicesĀ 

  • Performing procedures such as joint injections and dermatologic proceduresĀ 

  • Coordinating care with physicians, APP colleagues, behavioral health professionals, care coordinators, social workers, psychologists, pharmacists, and specialty providersĀ 

  • Building continuity through longitudinal care and established patient relationshipsĀ 

  • Contributing to a teaching environment that regularly welcomes medical students, nursing students, and other healthcare learnersĀ

ClinicalĀ 

  • Provides expert evidence-based, top-of-scope care in their clinical practiceĀ 

  • Acts as part of a specialty team, collaborating with other clinicians as appropriateĀ 

  • Provides health education to patientsĀ 

  • Maintains accurate, timely, and thorough documentationĀ 

  • Ensures compliance with institutional and regulatory standardsĀ 

LeadershipĀ 

  • Supports development of best practices and protocols in specialtyĀ 

  • Serves on committees and workgroups to enhance patient careĀ 

  • Provides education to learners in the clinical spaceĀ

MINIMUM REQUIREMENTS Education: Graduate of an accredited graduate school of nursing or physician assistant program with a master’s degree.

Experience: Department and unit specific.

Licensure: License or license eligible as an Advance Practice Provider (Nurse Practitioner, Physician Associate, Certified Registered Nurse Anesthetist, and Certified Nurse Midwife) as set forth by the state of Virginia. If Nurse Practitioner, license to Practice as a Registered Nurse in the Commonwealth of Virginia is required. Must demonstrate and maintain all credentialing and licensing requirements of UVA Health System, the Virginia Board of Medicine or the Virginia Board of Nursing, and appropriate certifying bodies. Current Basic Life Support (BLS) required. Current PALS/NRP, as required, specific to position. Advanced Cardiac Life Support (ACLS) as required, specific to position.

PHYSICAL DEMANDS

Job requires standing for prolonged periods, frequently bending/stooping, reaching (overhead, extensive, and repetitive); Repetitive motion: computer keyboard. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 50 - 100lbs. May be exposed to noise, radiation, radioactive materials, blood/body fluids and infectious disease.

The pay range for this role is $115,830.00 - $171,163.20 annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
