Job Summary 

CAPS & Maple City is an organization that supports healthy, safe, and thriving children and families by providing quality, comprehensive services to all.     

The Health Care Navigator contributes to the mission and values of CAPS & Maple City by supporting patients and non-patients in accessing affordable health care.  


Duties and Responsibilities 

  • Assists patients in accessing insurance information and coverage through Medicaid and the Health Insurance Marketplace. 
  • Maintains certification as an Indiana Navigator 
  • Assist patients in accessing in-house affordability resources. 
  • Assist patients access health care financial assistance programs through Goshen Health 
  • Participates in community events to educate people about CAPS & Maple City’s navigator services 
  • Maintains a high level of accuracy, completeness, confidentiality, and integrity in all documentation. 
  • Documents patient contact using electronic medical record 
  • Documents patient interactions and services provided in the electronic medical record (EMR 
  • Utilizes telephone encounters, encounter notes, and other appropriate EMR documentation methods to accurately capture patient contacts and services. 
  • Performs other duties that support shared team goals and organizational priorities, as assigned. 
  • Coordinates and follows up with members of the care team as needed to support patient access and continuity of care.

Requirements

Required Skills and Abilities 

  • Experience working with diverse groups of people  
  • Ability to take initiative, work independently, and collaborate with the team  
  • Ability to respectfully translate a variety of real-life situations into the Medicaid framework 
  • Fluently bilingual: English/Spanish; oral and written  
  • Effective interpersonal skills 

Education and Experience 

  • Associate degree strongly preferred, especially in social work or related field 
  • Certification as an Indiana Navigator  
  • Certified Application Counselor required (Training provided)

  • Ability to perform prolonged periods of time in front of a computer
