Join Our Therapy Team

Garden City Health Care Center is seeking a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team on a part-time basis. This role is ideal for a COTA who is passionate about helping residents maximize independence and quality of life in a skilled nursing and rehab setting.

General Purpose

The Certified Occupational Therapy Assistant (COTA) assists the Occupational Therapist in providing patient-centered therapy services, carrying out treatment plans, and documenting resident progress in accordance with physician orders, state practice acts, and facility policies.

Essential Duties and Responsibilities

  • Treat residents as directed by the Occupational Therapist and physician-approved plan of care

  • Provide direct patient care to improve functional independence and quality of life

  • Record daily treatment notes and weekly progress notes in accordance with OT Board and payer requirements

  • Communicate resident progress, challenges, and plans with the rehab and interdisciplinary teams

  • Assist with discharge planning and caregiver education

  • Instruct residents, families, and nursing staff on maintenance programs and safety techniques

  • Assist nursing department with training of Restorative Aides as needed

  • Participate in rehab meetings, care conferences, and in-services as assigned

  • Assist with cleaning, maintenance, and organization of the therapy treatment area

  • Report equipment issues promptly to ensure safe operation

  • Maintain positive, professional relationships with residents, families, and staff

Supervisory Responsibilities

  • This position has no supervisory responsibilities

Qualifications

Education & Experience

  • Graduate of an accredited Occupational Therapy Assistant program

  • Skilled nursing or geriatric experience preferred but not required

Licensure & Certifications

  • Current, active California Certified Occupational Therapy Assistant (COTA) license in good standing

  • Must meet all continuing education requirements per state regulations

Skills

  • Proficient in electronic documentation systems (Casamba and PointClickCare preferred)

  • Strong communication, time management, and clinical skills

  • Ability to work independently and as part of a collaborative team

Physical Demands

  • Frequent standing, walking, reaching, pushing, and pulling

  • Ability to lift and move up to 50 pounds frequently

  • Occasional stooping, kneeling, crouching, or balancing

  • Visual abilities include close, distance, color, peripheral, and depth perception

  • Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions

Work Environment

  • Skilled nursing and rehabilitation setting

  • Noise level typically low to moderate

  • One primary work location
