POSITION: Lead Caregiver

REPORTS TO: Executive Director/House Manager

JOB SUMMARY: Assist the Executive Director in the implementation and monitoring of all the CBRF operations. Act as a support mechanism between the staff and the Executive Director/House Manager regarding all Resident, family, and community issues. Maintain the care, dining, and life enrichment philosophies. Be creative and flexible. Must work every other weekend, but willing to cover all shifts. Perform hands on care to the Residents. Provide a safe, secure, and loving home for all the Residents. Represent the Executive Director/House Manager in his or her absence. Assist in staff development and continuing education.


DUTIES:

The Lead will be responsible for the below job duties including but not limited to: 

  • Represent New Day with professionalism, always embracing New Day’s culture, which includes “treating others as you would like to be treated.”
  • Assist the Executive Director/House Manager with all Resident needs.
  • Participate in the manager on call rotation per the schedule set by Executive Director/House Manager.
  • Responsible to assist with coverage of extra shifts when replacements cannot be found.
  • Be supportive, professional, and positive with the Executive Directors/House Manager’s decisions.
  • Represent the Executive Director/House Manager in his or her absence and communicate with families and community partners (i.e. pharmacy, therapy, hospice, emergency services)
  • Help coordinate transportation involving all off-site functions and Resident doctor appointments.
  • Assist Executive Director/House Manager with monthly State/Company monitoring (checklists)
  • Maintain open communication with family members.
  • Monitor shift duties daily, provide input and encouragement to team members.
  • Assist residents in activities of daily living (i.e., toileting, bathing, eating, etc. per individualized service plan
  • Must be able to assist semi-ambulatory residents with transfers and personal care, which may, at times, require lifting and gait-belt use.
  • Assist in maintenance of physical community on a daily basis (i.e., cleaning, straightening, laundry, dishes etc.)
  • Cook meals and provide snacks for the residents, following the menu plan.
  • Communicate needed supplies and equipment to the Executive Director/House Manager.
  • Monitor health of Residents and notify Executive Director/House Manager of any changes.
  • Monitor and chart administration of residents’ medications as applicable.
  • Document accidents and incidents using incident reporting form. in electronic record and report all incidents or observed concerns(conflict, potential misconduct etc.) to Executive Director /Designee.
  • Document resident daily observations (i.e., daily activities, behaviors, changes, etc.)
  • Facilitate daily life enrichment programs, in accordance with the life enrichment schedule.
  • Provide comfort and compassion to the residents, treating all residents with dignity, warmth and respect with reference to Resident’s Rights.
  • Communicate concerns to the Executive Director/Nurse/designee immediately.
  • Call 911 if resident is in distress 
  • Follow emergency procedures if an emergency occurs.
  • Be available to attend mandatory monthly staff meetings and training as requested by the Executive Director/House Manager and to achieve regulatory required education compliance.
  • Be available for occasional hours at other New Day Communities.
  • Responsible for reading any new or updated ISP’s on an as-needed basis.
  • Responsible for clearing all walkways of snow, ice, or any other debris.
  • Follow all guidelines according to DHS 83.
  • All other duties as assigned by the Executive Director/House Manager.

Requirements

QUALIFICATIONS: Must complete probationary period successfully. Must be able to be flexible and work alone if necessary. Should be eager to learn. Must have a history of actively demonstrating New Day’s culture and exhibiting extraordinary care toward New Day’s residents. Must have a history of compliance with Frontida’s policies and procedures. Desired: High School Diploma or equivalent, CBRF Training completed.


PHYSICAL REQUIREMENTS:

  • Stand, sit and walk for long periods of time.
  • Full ranges of motion to climb, balance, kneel, crouch or crawl.
  • Full function of hands and arms
  • Occasionally lift/or move up to 25 pounds with occasional lifting of residents at higher weight.
  • Able to transport residents in wheelchairs.
