Team 2: Atlantic Beach , Baldwin, Cedarhurst, East Rockaway, Bellmore, Freeport , Hewlett, Inwood, Island Park, Lawrence, Levittown, Lido Beach, Long Beach, Lynbrook, Merrick, Oceanside, Point Lookout, Rockville Centre, Seaford, Valley Steam, Wantagh & Woodmere.Â
- 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Job Details
Our Home Care LPN’s:
- Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
- Measure vital signs, Administer IVs, change dressings, clean wounds as needed
- Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
- Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
- Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
Minimum Qualifications:
- New York State Licensed Practical Nurse (LPN) license
- Minimum of one year of recent hands on experience
- Basic Life Support Certification
