Concordia Visiting Nurses is seeking a dedicated and adaptable Triage Licensed Practical Nurse to join our team! As a Triage Licensed Practical Nurse you’ll play a key role in coordinating and overseeing patient care. You’ll have the opportunity to work in a supportive and collaborative environment, making a meaningful impact on patient outcomes and the well-being of our community. This is a 32 hour per month part-time/casual position.  We value flexibility and work-life balance, so the role allows for some variation in hours to meet both the needs of our patients and your personal schedule
Qualifications for the triage Licensed Practical Nurse:
- Licensed Practical Nurse with a current valid Pennsylvania license
- Knowledge of OASIS [Outcome and Assessment Information Set] guidance required
- Minimum of two [2] years of current experience as a professional nurse in a medical/surgical environment and one [1] year of home health experience preferred
- Reliable, independent means of transportation. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state.

What you will do as the triage Licensed Practical Nurse:
- Practices within the established scope of practice for a Licensed Practical Nurse (LPN) in accordance with applicable state regulations, organizational policies, and procedures.
- Reviews and approves the referral for accuracy and completeness
- Reviews and processes clinical workflow tasks
- Triages patients using telecommunications methods, including telephone and HIPAA-compliant virtual communication platforms (e.g., secure Microsoft Teams meetings or other approved telehealth systems), to assess patient needs and determine the appropriate level of care. The position will communicate clinical findings, patient concerns, and care recommendations to physicians and interdisciplinary staff to ensure timely, coordinated care.
- Assesses patient signs and symptoms via telecommunications, monitors responses to treatments/medications, and reports significant physical or emotional conditions to the physician.
