Summary of Position


The Lead Nurse plays a critical role in the delivery of quality medical care and leadership with in the center. This position requires a person with strong clinical skills along with assessment and communication skills. The Lead Nurse provides care as described by the established policies and procedures of RCCHC. This position will provide clinical care in collaboration with other partnerships to achieve positive patient outcomes. The Lead Nurse will model behaviors consistent with the published values and the Code of Conduct of RCCHC.

Population Served


Roanoke Chowan Community Health Center will provide open access, community-oriented primary care of the highest quality to all residents within our service area and surrounding counties, regardless of their ability to pay.


Minimum Qualifications

 

  • Must demonstrate competencies in nursing practice
  • Works effectively with others and is able to organize workload, as well as assume responsibility 
  • Must be able to work independently, exercise appropriate decision making skills, and function effectively on a team
  • Must be able to speak and communicate clearly and effectively


Education: BSN preferred

Experience: At least two years primary or ambulatory care preferred, at least one year supervisory experience preferred


Certification(s)/Licensure: BLS certification required. Current license as a RN in the state of North Carolina


Knowledge:

  • Knowledge and thorough understanding of Federally Qualified Health Centers (FQHC’s) in regard to the Federal 330 Public Health Service Act
  • Knowledge of thorough understanding of the 340B program
  • Knowledge of computer systems and applications.

Essential Functions


  • Serve as a resource and first point of contact for clinical team members
  • Plans for and institutes practices that promote the organizations capacity to provide resources that ensure continuity of care among primary care, specialties, other disciplines and across health care systems.
  • Evaluates and manages the performance of nursing personnel and outcomes of nursing care delivery systems. Monitors staff performance with feedback given on a regular basis.
  • Initiates activities that focus on improving the quality of clinical nursing practice and nursing care delivery system.
  • Adheres to regulatory, ethical and professional standards in business and care delivery practices. Role models ethical practices in patient care and business situations.
  • Addresses conflict openly, using tactful, sensitive measures to mediate mutually beneficial resolutions.
  • Sets standards, serves as a role model, and set expectations for conducting and participating in performance improvement initiatives
  • Assures that initial and ongoing competency assessments occur within organization-specific time frames using measurable performance criteria. Determines and documents nursing staff competency before assigning them to patient care responsibilities.
  • Perform independent nursing functions using RCCHC established procedures, policies, and guidelines.
  • Works with Nurse manager to ensure effective staffing and flow of provider team on an ongoing basis
  • Works with Nurse Manger and Practice Administrator for effective communication to maximize patient care and satisfaction
  • Fill in as needed for the clinical team to ensure effective workflows and patient care 
  • Maintain proper storage and handling of equipment, supplies, and medications
  • Participate as an active member of the RCCHC team
  • Maintain compliance with workflows, policies and procedures as well as other activities to support PCMH model of care and accreditation
