PURPOSE             

At National Church Residences, a Transition Coordinator works from home and travels to meet their clients where they are. They are responsible for facilitating the steps needed to assist an individual from a long-term care facility to a home setting in the community. As a Supervisor of Transition Coordinators, you train and ensure that Transition Coordinators within an assigned area (Western Ohio) are carrying out the aforementioned policies and procedures relating to the transitioning of individuals from long term care to community settings.

ESSENTIAL FUNCTIONS

  • Serves as a liaison between the health care and housing professionals, including, but not limited to the HOME Choice Transition Coordinator (TC), service coordinators, discharge planners, home/community-based service representatives. Is knowledgeable on all service lines including adult day health, personal care/private duty, service coordination, skilled care and hospice. Works closely with all managers of all service lines.
  • Provides guidance, training and mentorship for new hires and existing staff, including routine audits with documented assistance for improvement when needed.  Monitoring for compliance with HOME Choice contract requirements and compliance to timelines. Serves as a liaison to the TC and community, housing and/or health care providers to assist with a transition
  • Completes staff performance evaluations including corrective action if needed
  • Serves as a liaison to community agencies, network with community service providers, and seek
    out new services available to individuals benefiting from the HOME Choice program
  • Monitors the ongoing provision of services from community agencies and subcontractors, ensuring adherence to contract requirements.
  • Function as a Transition Coordinator and manage a caseload as needed.
  • Provides supervision and support to transition coordinators in an assigned region.

NON-ESSENTIAL FUNCTIONS

  • Assist with presentations to educate internal and external stakeholders on HOME Choice Program
  • Distribute feedback reports based on surveys, monitors, and other measurement tools;
  • Oversees appropriate, accurate and timely payroll submission, HR, A/P, A/R, and billing procedures. Ensures appropriate staffing levels. 
  • Assists with linkage to employment options if participant interest is indicated.

  • With respect to Client Rights, ensures all care is provided with respect and dignity for clients, reports all complaints made by clients and/or families to the appropriate supervisors, reports all allegations of abuse, misappropriation of funds/client property and/or any other corporate compliance items immediately.  Advocate individuals’ personal preferences and right to self-determination.
  • Must adhere to all policies, procedures, terms and conditions set forth in the National Church Residences’ Employee Information Guide (EIG) as well as any facility handbook including but not limited to corporate compliance, drug free workplace, HIPAA training safe work practices, all federal, state, local regulations and laws. 
  • Performs other duties as assigned. 

Education:

Bachelor’s Degree in social work or a related field from an accredited institution is required.

Experience:

Minimum of 3 years in social service delivery with the elderly and/or family populations including but not limiting to chronic health conditions, mental health and/or substance use, and physical disabilities. Demonstrated working knowledge of supportive services and other resources in the area served by the project.

Skills:

  • Must have the ability to understand data.

  • Must have excellent communication skills; able carry out verbal and written instructions to others.

  • Able to occasionally lift up to 50lbs

Travel: Frequent, required to be available on a daily basis via automobile throughout the Western half of Ohio.

Licensure: Valid driver’s license and able to meet National Church Residences’ motor vehicle policy.
