Case Manager — Medical Readiness Support

Location: Columbus, Ohio
Client: Ohio Army National Guard (OHARNG)
Worksite: Defense Supply Center Columbus, Medical Detachment
Schedule: Monday–Friday, generally between 7:30 a.m. and 4:30 p.m. Eastern Time

Position Overview

Crossroads Talent Solutions is seeking an experienced Case Manager to provide non-clinical medical case management support to the Ohio Army National Guard. The Case Manager will assist in maintaining the medical readiness and deployability of Service Members by managing assigned medical cases, maintaining accurate case information, coordinating documentation and follow-up activities, and communicating case status to Soldiers, units, Medical Readiness NCOs, and State Surgeon’s Office personnel.

The position supports the management of individual Soldier medical cases using Army medical-readiness systems and requires careful handling of medical documentation, personally identifiable information, and protected health information. The PWS requires services to support approximately 9,500 Service Members statewide.

Key Responsibilities

  • Manage and maintain assigned non-clinical medical readiness cases from initiation through appropriate disposition.
  • Process medical information and maintain accurate case records using required Government systems, including eCase, MODS, MEDCHART, MEDPROS, and HRR.
  • Ensure medical information is processed in accordance with HIPAA requirements and applicable privacy standards.
  • Obtain, process, maintain, and track Release of Information (ROI) requests for assigned Service Members.
  • Follow up with civilian medical providers when requested records have not been received within established timelines.
  • Prepare comprehensive status information for open cases in advance of bi-monthly case reviews.
  • Conduct medical pre-screen support and process initial medical documentation for mobilizing Soldiers participating in Soldier Readiness Processing events.
  • Monitor and track scheduled medical appointments and outstanding medical-readiness requirements.
  • Provide medical-readiness and compliance information to designated Battalion, Brigade, and State Surgeon’s Office personnel.
  • Compile information concerning outstanding medical and dental issues and support required metrics and reporting.
  • Process medical and dental documentation and route profile-related concerns to the appropriate designated authority.
  • Maintain regular follow-up communication with Soldiers, units, Medical Readiness NCOs, and State Surgeon’s Office personnel to support case progression and follow-up care.
  • When assigned, assist with documentation supporting Service Members entering the Integrated Disability Evaluation System (IDES) and Medical Evaluation Board processes.
  • Maintain accurate, complete, timely, and confidential case documentation in accordance with Army Regulations, Department of Defense Instructions, and applicable personnel policy guidance.

Required Qualifications

Candidates must meet the Case Manager qualifications identified in the PWS through one of the following pathways:

  • Possess current Ohio credentials as a Licensed Practical Nurse (LPN); or
  • Possess current Ohio credentials as a Direct Service Social Worker; or
  • Hold a bachelor’s degree in a health-related field with at least one year of professional case management experience.

Knowledge, Skills, and Abilities

  • Strong case-management, documentation, organization, and follow-up skills.
  • Ability to manage multiple active cases and competing deadlines.
  • Ability to communicate professionally with Service Members, military units, medical-readiness personnel, civilian providers, and Government staff.
  • Strong attention to detail when reviewing medical documentation and updating electronic records.
  • Ability to safeguard sensitive medical information and Personally Identifiable Information.
  • Ability to work independently while coordinating closely with Government and contractor personnel.
  • Ability to learn and effectively use Army medical-readiness databases and case-management systems.

Security and Training Requirements

Personnel requiring access to DoD networks and systems must satisfy applicable Government access and background requirements. The PWS also identifies required training associated with information assurance, operations security, controlled unclassified information, HIPAA/Privacy Act, PII protection, and medical-readiness systems.
