Our Weight Management and Metabolic Health office is seeking a Full-Time Licensed Practical Nurse (LPN) to join our dedicated, patient-focused team in Clifton Park. This role is central to supporting patients throughout their Weight Management and Metabolic health journey, collaborating closely with Behavioral Health providers, Registered Dietitians, and Advanced Practice Providers to deliver comprehensive care focused on lifestyle modification, medical weight loss, and long-term metabolic wellness.
**This position is eligible for up to a $3,500 recruitment and retention bonus!!**
Schedule:
  • 40 hours per week
  • Monday–Friday, 7:30am – 4:00pm
Key Responsibilities:
  • Room patients and support providers during Weight Management and Metabolic Health visits
  • Assist with care related to metabolic conditions, lifestyle modification, and medical weight loss programs
  • Manage and work from multiple task bins related to patient care, authorizations, lab results, and follow-ups
  • Document verbal conversations with patients when appropriate
  • Accurately and timely document clinical information in the patient record
  • Maintain efficient patient flow in a fast-paced specialty practice
  • Prepare patients for visits, including vitals, medication reconciliation, and program-specific or metabolic health assessments
  • Perform accurate and timely order entry into the laboratory system
  • Process and manage patient prescriptions, including those related to weight and metabolic health treatment plans
  • Discuss test results and care plans with patients per provider instructions
  • Collaborate closely with providers and care team members to support patient engagement, education, and success in weight management and metabolic health programs

Compensation: $26.00 - $33.00 per hour

Qualifications
  • Valid NYS LPN license.
  • Previous LPN experience in a medical practice or prior experience as a Medical Assistant or Patient Care Tech.
  • Strong computer skills with experience using EMR.
  • Knowledge of basic nursing principles and protocols.
  • Knowledge of policies and procedures related to infection control, environmental safety, and patient confidentiality.
  • Maintain strictest confidentiality and discretion.
  • Knowledge of medical chart reporting/records.
  • Skill in appropriate assessment and assistance techniques.
