Job DetailsJob Location: 487 - HQ - Chattanooga, TN 37402Position Type: Full TimeEducation Level: High SchoolTravel Percentage: NoneJob Shift: DayTextCare Pro Practice Coordinator Reports to: TextCare Pro Clinical Director Location: Onsite Chattanooga, TN  Schedule: M-F Standard Business Hours   Position Summary: The TextCare Pro Practice Coordinator is responsible for day-to-day operations of TextCare Pro, ensuring that TextCare Pro patients move through the appropriate wellness and care workflows without falling through the cracks. This includes ensuring outreach occurs, visits and follow-ups are completed, and high-risk patients receive the ongoing attention they require. The Practice Coordinator directly oversees the TextCare Pro Coordinators and works closely with providers, the Clinical Director, Account Management, and Operations to keep the practice running effectively. This role owns workflow execution, patient follow-through, team performance, and provider capacity to ensure patients receive timely, coordinated care. Key Responsibilities: Practice & Team Management Serve as the direct manager and primary point of contact for TextCare Pro Coordinators, providing day-to-day direction, coaching, support, and issue resolution, escalating issues to Clinical Director as appropriate. Manage TextCare Pro Coordinator performance, including ongoing feedback, development, performance reviews, and performance management, escalating significant concerns to the Clinical Director as appropriate. Direct Coordinators and providers as needed to ensure patient care, outreach, and follow-up work is completed in a timely manner. Wellness Visit Completion Own the TextCare Pro wellness workflow from outreach to completion, ensuring every patient identified for wellness either completes the visit or has appropriate outreach attempts. Monitor outstanding wellness visits and direct TextCare Pro Coordinators to follow up with patients who remain unscheduled, have not completed or responded to outreach. Identify and address barriers to wellness visit completion, coordinating with the Clinical Director, Account Management, or Operations as needed. Patient Follow-Up & High-Risk Patient Management Ensure no patients fall through the cracks with an emphasis on patients identified as high-risk or those needing additional follow-up. Ensure patient encounters are reviewed, follow-up needs are identified, and appropriate actions are assigned, completed and documented appropriately. Ensure patients identified as high risk receive timely and persistent outreach, with appropriate follow-up until they engage in care or reasonable outreach efforts have been exhausted.  Help maintain consistent processes and adherence to applicable privacy, security, and organizational requirements.   Required Qualifications: 2+ years of experience in practice management, healthcare operations, practice coordination, healthcare administration, care coordination, or a related role. Ability to work an onsite/hybrid role in Chattanooga, TN Ability to work standard business hours M-F  Strong organizational and project management skills with the ability to manage multiple accounts, workflows, and priorities simultaneously. Strong judgment and demonstrated ability to independently prioritize, troubleshoot, and resolve operational issues. Excellent communication and interpersonal skills with the ability to work effectively across clinical, operational, and client-facing teams. Comfortable using technology, systems, and operational data to identify issues and improve consistency and efficiency. Preferred Qualifications: Prior experience with direct people management, coaching, or performance management.   Qualifications
