MercyOne Des Moines Medical Center: Full-Time (80 hours biweekly); Day Shift

POSITION PURPOSE:
The Social Work Coordinator is responsible for focused ownership and coordination of acutely hospitalized patients with highly complex medical, psychosocial, behavioral, and system‑driven needs. Embedded within the Acute Care Management team, this role provides sustained attention, advanced care coordination, and accountability for complex inpatient populations, partnering closely with care teams and leadership to remove barriers to progression, support timely discharge planning, and improve patient outcomes. While the role does not assume ownership of outpatient settings or teams, it is accountable for ensuring intentional coordination, connection, and effective handoff to appropriate outpatient, post‑acute, and population‑based resources as part of transitions of care. Adheres to MercyOne’s performance expectations and upholds the Mission, Vision, and Values of MercyOne.

ESSENTIAL FUNCTIONS

  • Knows, understands, incorporates, and demonstrates the Trinity Health and MercyOne Mission, Vision, and Values in behaviors, practices, and decisions.
  • Collaborate with interdisciplinary care teams, including physicians, nursing, care management, utilization review, post-acute providers, and community partners to ensure coordinated, patient-centered care.
  • Provide expertise and support in addressing complex psychosocial, behavioral, social determinant of health, and care transition challenges.
  • Facilitate effective communication among patients, families, care teams, and external stakeholders to promote informed decision-making and successful care outcomes.
  • Apply knowledge of healthcare regulations, payer requirements, organizational policies, and community resources to support patient care and care progression

MINIMUM QUALIFICATIONS

  • Must possess a comprehensive knowledge of Social Work as normally obtained through a Bachelor’s degree in social work or a related field, and three (3) years of experience working with complex, high risk patient populations in an acute or inpatient setting.  
  • Master’s degree in related field preferred. Current Iowa license in Social Work. (MercyOne Genesis - current Social Work license in either Iowa or Illinois is required at the time of hire.
  • A secondary Social Work license for the other state must be obtained within 12 months of the hire date.)
  • Proof of completion of Mandatory Reporter – Child and/or Dependent Adult Abuse training within three (3) months of hire. Colleagues employed and working solely in facilities located in Illinois are exempt from this requirement in accordance with applicable state requirements.
  • Must be comfortable operating in a collaborative, shared leadership environment.
