About the Role:
NorthStar Home Hospice LLC is looking for a dedicated and detail-oriented Hospice Quality Assurance & Performance Improvement professional to join our compassionate care team in Cincinnati, OH. This is a meaningful opportunity to make a lasting impact by driving clinical excellence and ensuring the highest standards of care for patients and families during life's most important moments.
Responsibilities:
- Develop, implement, and oversee the Quality Assurance and Performance Improvement (QAPI) program in compliance with hospice regulations and standards
- Monitor and analyze clinical and operational data to identify trends, gaps, and opportunities for improvement
- Lead performance improvement initiatives and coordinate interdisciplinary team involvement in quality projects
- Conduct audits of clinical documentation, patient records, and care plans to ensure regulatory compliance
- Prepare and present quality reports and metrics to leadership and clinical staff
- Ensure adherence to CMS Conditions of Participation and state hospice regulations
- Collaborate with clinical staff to develop and deliver education related to quality standards and best practices
Requirements:
- Current RN or clinical licensure in the state of Ohio required
- Prior experience in hospice, home health, or palliative care quality assurance or compliance
- Prior management experience in hospice, home health or pallaitive care a plus
- Strong knowledge of CMS hospice Conditions of Participation and QAPI regulations
- Experience with clinical auditing, data analysis, and performance improvement methodologies
- Excellent organizational, communication, and critical thinking skills
- Proficiency with electronic health records (EHR) and quality reporting tools
