Mercy Medical Center Home Health and Hospice

Schedule: Tuesday-Friday 10:00PM - 8:00AM


 

How you will make a difference!!
 

As a Hospice LPN with Mercy Medical Center Home Health & Hospice, you will provide compassionate, skilled nursing care to patients in their homes while supporting patients and families throughout their hospice journey. You will work closely with the RN and interdisciplinary team to promote comfort, safety, and quality of life.
 

KEY RESPONSIBILITIES

  • Provide skilled nursing care according to the patient’s plan of care and physician orders.

  • Monitor patient condition and report changes to the RN.

  • Administer and reconcile medications as prescribed.

  • Educate and support patients and families regarding disease progression and comfort care.

  • Complete accurate clinical documentation in accordance with agency and regulatory requirements.

  • Communicate with the care team regarding patient needs and changes.

  • Participate in on-call responsibilities, case conferences, required training, and meetings.

  • Follow all agency policies, procedures, and applicable regulations.

     

QUALIFICATIONS

  • Current LPN license in the state of employment.

  • One year of LPN experience strongly preferred; hospice experience preferred.

  • Strong communication and interpersonal skills.

  • Compassionate, patient-centered approach to care.

  • Ability to work independently, prioritize tasks, and make appropriate nursing judgments.

  • Ability to work effectively as part of an interdisciplinary team.

  • Valid driver’s license and automobile insurance with your name listed as a driver.

     

EDUCATION & CREDENTIALS

  • Graduate of an approved practical nursing program.

  • Current, unrestricted LPN license.
