Job DetailsLevel: ExperiencedJob Location: Canandaigua , NY 14424Position Type: Full TimeEducation Level: High SchoolSalary Range: $22.00 - $24.00 HourlyJob Category: Health Care / CounselingJob Summary:  In accordance with the organization’s mission, vision and values, the Supportive Living Counselor Assistant is responsible for case management services for individuals who have been accessed as needing addictions services.  The following duties will apply: Duties Responsible for screening, orientation, case management and basic crisis response. With intensive supervision, will conduct intake, referral and care coordination. Demonstrates teamwork and an ability to work in a team environment with coworkers and supervisors.  Demonstrates the ability to be flexible and cooperative.       Co-facilitates community meetings and group support sessions for residents.  Responsible for completion of discharge activities according to policies, practices and procedures.  Assures all documentation is completed in a timely and accurate manner in compliance with agency/OASAS regulations, practices, policies & procedures.  Demonstrates professionalism by providing quality service and care with respect and dignity for the clients and as a representative of FLACRA.  Participate in general, staff, in-service and supervisory meetings held by the Agency. Perform other duties and functions as directed.  QualificationsMinimum Qualifications:  High School Diploma or GED. Valid NYS Drivers’ License. Pursuit of QHP status is encouraged.      Other Skills/Knowledge and Experience Excellent written and verbal communication skills. Demonstrated ability to communicate effectively and work cooperatively with culturally diverse persons, staff and community service providers. Knowledge of local behavioral health system. Ability to multi-task, have good problem solving and time management skills and the ability to remain calm in crisis. Proficient in E-record.
