The primary focus of this position will be to coordinate referrals from the time of referral until the referral is closed (not accepted) or admitted into a residential program.   DUTIES AND RESPONSIBILITIES Engages and communicates effectively with consumers, family, referral sources, and team members. Receives referrals and initiates referral form in electronic health record. Evaluates referral based on ASAM criteria to determine appropriateness for admission. Notifies referral source/consumer within 72 hours of receiving referral regarding the status of the referral. Sends updates on accepted referrals weekly. Schedules admissions and coordinates time of admission with program and administrative staff. Assists in completing the first MNS/care connection for new admissions. Speaks effectively in group situations to assist with marketing the program. Documents all activities electronically within 24 hours of service. Attends all departmental and center meetings/trainings as assigned. Writes professional, quality reports, and correspondence. Adheres to confidentiality, HIPAA, and risk management policies and procedures including but not limited to completing Incident Reports. All other duties as assigned.    SUPERVISORY RESPONSIBILITIES None.Qualifications
