What You'll Do

As a Case Manager, you will:

• Work as part of a multidisciplinary ACT team providing holistic, recovery-focused services to participants
• Assist individuals in developing and achieving goals outlined in their recovery plans
• Connect participants to community resources, educational opportunities, healthcare services, and supportive programs
• Advocate for participants to ensure access to needed services and supports
• Develop and maintain relationships with community agencies and service providers
• Provide community-based services throughout the Lehigh Valley service area
• Transport participants to appointments and community activities when needed
• Complete assessments, service documentation, and reports in accordance with organizational and regulatory requirements
• Identify service gaps and recommend strategies to enhance participant outcomes
• Support participants in achieving greater independence, stability, and quality of life

Qualifications

Required

• Two years of experience supporting individuals in behavioral health, addiction recovery, homeless services, or BHH/IDD programs
• Intermediate computer skills and ability to complete electronic documentation
• Strong interpersonal, communication, and customer service skills
• Ability to work independently and as part of a multidisciplinary team
• Valid driver's license and ability to travel throughout the community as required

Preferred

• Bachelor's Degree in Human Services or a related field
• One year of case management experience
• Experience working with individuals experiencing behavioral health challenges
• Knowledge of community resources and recovery-oriented services

Schedule

• Full-Time Position
• Community-based work throughout the Lehigh Valley region
• Schedule flexibility required based on participant and program needs
• Some evening, weekend, and location flexibility may be required

Physical Requirements

• Ability to lift up to 50 pounds occasionally
• Ability to sit, stand, walk, bend, reach, kneel, crouch, and climb stairs as needed
• Ability to travel regularly throughout the community
• Ability to perform essential job functions with or without reasonable accommodation
